The Bristol Health Diabetes Program offers the latest in medical care and comprehensive self-management education and training for people with diabetes. Because diabetes is a complicated disease, our program teaches you skills to prevent diabetes-related complications, such as blindness, kidney and heart disease, stroke and amputations. Our goal is to help you “live well” with diabetes.
When looking for a program, choose one that is staffed with a Diabetes Care and Education Specialist, has access to a board-certified Endocrinologist and is recognized by the American Diabetes Association. It is comforting to know that the Bristol Health Diabetes Program meets all of these standards.

How Does this Program Work?
Before we can see you, a physician referral is required. When you begin our program, we will work with you to develop a teaching plan tailored specifically for you and we will help you set goals that will make living with diabetes more manageable. Feel free to bring a family member with you to your appointments.
